Nautilus Roadshow Success for Healthcare Company Vygon (UK) Ltd



Intravenous products: Vygon (UK) Ltd ran a series of Nautilus Roadshows at the beginning of June in London, Birmingham and Manchester. We have had some great customer feedback and we were fortunate to have three expert speakers.
Mauro Pittiruti (Catholic University, Rome, Italy) gave an overview of the technology and the research behind Nautilus, and shared his vast experience with this technique. Gemma Oliver (Matron for Vascular Access and Hospital at Home, East Kent Hospitals NHS Trust) and Elizabeth Simcock (Clinical Nurse Specialist, Central Venous Access, University College London Hospitals NHS Foundation Trust) then showed how Nautilus can be incorporated into PICC placement practice in hospitals in the UK. They have found that through the use of Nautilus ECG technology to perform real-time PICC tip positioning, they can eliminate the use of X-ray, enhance the patient journey, and save the hospital time and money. In summary the Roadshows have helped establish that:
  • Through adopting a proper technique and using a proper ECG monitor, in well-trained hands intra-cavitary ECG (IC-ECG) is a safe, accurate and cost-effective method for verifying the position of the tip of any central VAD in the vast majority of patients.
  • IC-ECG will become the standard procedure for assessing tip position, much as ultrasound guidance has become the standard procedure for central venepuncture.
  • Post-procedural chest X-ray becomes unnecessary in most cases, when IC-ECG is coupled with ultrasound guidance.
  • Materials DO make the difference. A dedicated ECG monitor (Nautilus) and high quality connections increase the feasibility, accuracy and simplicity of the IC-ECG method.
Paul Allen, the Vygon Business Development Manager who ran the Roadshows, said “The best thing that I will take away from these events is that X-ray may not even be a Bronze standard let alone Gold when compared to ECG-guided tip location. We had already realised this but it was good to see the theory reinforced by experts in their field.”
In other news about Nautilus, following its launch in early 2013, Vygon has successfully introduced the system at a south coast NHS Trust.
Vygon is now working with their Vascular Access team to ensure that all users have been fully trained in the use of Nautilus.
A spokesperson for the Trust said “We’re really looking forward to using Nautilus during PICC placement. Although it’s a relatively new concept the benefits are clear. Nautilus is going to change the way we place PICCs, which will benefit both patients and staff. Best practice is evolving and Nautilus is leading the way.”

MHRA suspends use of hydroxyethyl starch (HES) drips


 
The use of starch drips to treat critically ill patients and those undergoing surgery is to be suspended in the UK because their benefits no longer outweigh the risk of using them, the Medicines and Healthcare products Regulatory Agency (MHRA) announced today.
 
The drips are currently used in a range of clinical settings for treatment and prevention of low blood volume (hypovolaemia) and a steep drop in blood pressure (hypovolaemic shock), as well as for maintenance of adequate circulation during surgical procedures, should no longer be used.
 
Dr Sarah Branch, Deputy Director of the MHRAs Vigilance and Risk Management of Medicines Division said:
 
The use of these types of drips has fallen in the last year because of published evidence which shows that there is an increased risk associated with the use of hydroxyethyl starch products compared with simple salt solutions (crystalloids).
 
Having considered the available evidence, and taken advice from the Commission on Human Medicines, we have decided to suspend their use in the UK.
 
Professor Julian Bion, Dean of the Faculty of Intensive Care Medicine said:
 
'The decision to suspend the use of HES products, and the recommendation to use crystalloid solutions for fluid resuscitation, are supported by the Faculty of Intensive Care Medicine, the Intensive Care Society, and the Royal College of Anaesthetists. We have issued a position statement and guidance for our members on our respective websites.
 
The risks and benefits of starch drips are the subject of an EU wide review following the publication of studies which suggested an increased risk of renal injury and death in patients treated with these products compared with crystalloids (simple salt solutions).
 
The decision follows a recommendation by European Medicines Agency (EMA) and a review of the evidence by the MHRAs independent expert group the Commission on Human Medicines (CHM) - which found that the benefits of their use no longer outweighed the risks.
 
The MHRA will issue information to healthcare professionals to inform them of the decision and plans for the recall of the product from the supply chain.

Surgeons to be 'named and shamed' for blocking data from next week

The NHS will "name and shame" surgeons who do not consent to publish data on how their patients fare after surgery from next week, according to reports.

Under a new scheme aimed at improving transparency in the NHS, consultants face the publication of data on how their patients fared after treatment to help patients choose where to go for treatment.
Following the Telegraph's disclosure that doctors were being given the chance to opt out of the league tables Jeremy Hunt, the health secretary, announced last week that those who do so could be named publicly.
Now further details have emerged which indicate that those who attempt to hide information from patients under data protection laws could be listed online next week.
A report in the Health Service Journal claims that consultants who block the publication of their data will be named on a public website, most likely the Health care Quality Improvement Partnership, from 28 June.
Those who are named will have the chance to explain their decision by selecting one of a number of predefined reasons, such as concerns about the validity of the data or a moral objection.

A spokesman for HQIP told the journal: "The interest of the public in this pioneering work is clear and so to ensure patients are kept fully informed of progress regarding it, a list of consultants who have not consented will be published online."
The initial wave of data will cover ten surgical specialisms, but the transparency scheme will later be rolled out across the NHS.
More than 97 per cent of consultants covered by the first data release are believed to have consented, but last week there were still 92 individuals who had not agreed.
Professor Norman Williams, President of the Royal College of Surgeons, said: "We have been clear from the outset that no surgeon should use the law to hide the results of their surgery from the public.
"There are a small number of surgeons who have genuine reasons to withhold agreement, such as the quality of their data, and they must be open about those reasons."

Sagent Pharmaceuticals, Inc. announced the voluntary nationwide recall of three lots of Vecuronium Bromide for Injection 10mg (NDC number 25021-657-10) manufactured by Mustafa Nevzat Ilac Sanayii A.S. (MN Pharmaceuticals) and distributed by Sagent. Sagent has initiated this voluntary recall of Vecuronium Bromide for Injection to the user level due to the discovery of an elevated impurity result detected during routine quality testing of stability samples at the 18-month interval. The elevated impurity result has the potential to result in prolonged neuromuscular blockade for critically ill patients with renal failure.

HCPC Launches Consultation on Guidance for Professional Indemnity Cover and Registration

The Health and Care Professions Council (HCPC) has launched an eight week consultation to seek the views of stakeholders on guidance for registrants in relation to professional indemnity cover and registration.

The Government are proposing that all health professionals must hold professional indemnity cover as a condition of registration. This is subject to parliamentary approval and will apply to all of the professions regulated by the HCPC with the exception of social workers in England*. This is because these 15 professions are considered to be healthcare professions under the terms of the European Directive 2011/24/EU on cross-border healthcare.

The shocking local variation in early death rates has been exposed today by Health Secretary Jeremy Hunt to drive public awareness and boost council and NHS action to tackle public health problems.

The figures are part of Longer Lives, a new Public Health England (PHE) website which allows local people to see easily how their areas perform on early deaths from the major four killers, like heart disease and cancer, and how this varies across the country.
